What are common drug treatments for Alzheimer’s disease?

    Alzheimer’s disease

Alzheimer’s disease does not currently have a treatment option. There are no cures for Alzheimer’s disease once symptoms like memory loss and difficulties with learning, judgment, communication, and everyday living appear.

However, many people may benefit by getting symptomatic relief from certain medications. They can make the disease progress more slowly and keep the brain functioning well for longer. It is ideal to refer to a doctor and determine the best action. Often, people suffering from Alzheimer’s are recommended medicines based on their symptoms.

    What is Alzheimer’s Disease?

There is no cure or treatment for the brain condition Alzheimer’s disease. The illness profoundly impacts a person’s memory, thinking, learning, and organizational abilities, eventually affecting their capacity to perform everyday tasks. It is not an expected aspect of aging.

The symptoms aggravate with time. In fact, according to doctors, the illness process could last up to 10 years before Alzheimer’s disease symptoms start to show.

When memory issues become apparent, they are frequently classified as mild cognitive impairment (MCI). At this point, the brain adjusts to the effects of the disease, affecting intellectual function but leaving the ability to operate and live independently unaffected.

MCI may continue to progress in some persons at this point. However, dementia is most likely to progress in MCI patients. Some conditions may also bring on dementia, where daily functions like walking, sitting, and standing may be compromised.

Alzheimer’s disease patients who have advanced dementia are unable to communicate, recognize friends and relatives, or take care of themselvesPatients can choose to replace prescribed medicines with generic ones, and this saves cost.

    What causes Alzheimer’s?

Protein accumulation that is out of the ordinary in the brain leads to Alzheimer’s disease. These proteins, amyloid and tau proteins, accumulate and cause cell death.

Over 100 billion nerve cells and other cells make up the human brain. The nerve cells carry out all the communications required to carry out tasks like thinking, learning, remembering, and planning. The accumulation of amyloid protein, which forms bigger aggregates known as plaques in the brain cells, is thought to occur. Tau tangles are made of twisted tau protein fibers. These plaques and tangles obstruct nerve cell communication, stopping nerve cells from performing their functions. The symptoms experienced by people with Alzheimer’s are caused by the slow and continuous death of the nerve cells, which begins in one area of the brain (often in the area of the brain that governs memory) before spreading to other places.

    FDA-approved medicines for Alzheimer’s

The U.S. Food and Drug Administration (FDA) has authorized pharmaceuticals that fall into two categories —

    –
Those that may slow the advancement of the disease in Alzheimer’s patient

    –
Those that may momentarily lessen some of the symptoms of the disease.

There may be various treatment options (pill, patch or other). It is crucial to consult a healthcare practitioner before deciding on any course of treatment to ascertain whether it is necessary. A doctor with experience using these drugs should monitor patients and strictly follow the suggested guidelines.

Drugs in this class may slow the illness’s advancement, benefiting Alzheimer’s patients’ cognition and function.

    #1. Aducanumab (Aduhelm™)

Aducanumab is an FDA-approved drug that shows removing beta-amyloid. This is a protein whose aggregation, accumulation, and initiation is responsible for the development of Alzheimer’s Disease. Patients should discuss whether they are an ideal fit for a proposed treatment with a doctor before deciding on any action. Here, Aduhelm is a brand name with many alternatives in generics.

This chemical component in a medicine targets beta-amyloid, a tiny protein fragment that develops in the brain and builds into plaques. It is the key function of aducanumab. These plaques prevent nerve cells in the brain from communicating with one another and may also activate immune cells that cause inflammation and eat away at damaged nerve cells.

Amyloid plaques, one of the symptoms of the disease and one of its hallmarks, are one of the likely contributors, while scientists are unsure of what leads to death of cell and tissue loss along the course of Alzheimer’s. Aducanumab is the first treatment to show that eliminating amyloid, one of the symptoms of Alzheimer’s disease, from the brain has a chance of slowing cognitive and functional deterioration in persons with early Alzheimer’s. In the market, you will find medication brand names and generic drugs for Alzheimer’s. There are plenty of common generic medications as well.

    #2. NMDA antagonist

The FDA has approved memantine (Namenda®) for treating moderate to severe Alzheimer’s disease. Activating NMDA receptors can preserve the health of nerve cells. Generic drugs function differently from cholinesterase inhibitors. Memantine may be consumed on its own or with a cholinesterase inhibitor.

Memantine patients with moderate to severe Alzheimer’s disease outperformed placebo patients (A treatment-free control group in an experiment used as a benchmark to compare with treatment groups) in tests assessing daily activities like eating, walking, using the restroom, bathing, and dressing. Patients who function less well might gain the most by consuming this medicine.

Non-branded NMDA antagonist drugs are difficult to avail. Frequent consumption of such medicine may put the patient in a vicious cycle of buying expensive, branded medicines. Instead, you can walk into any Medkart shop and choose generic alternatives for Alzheimer’s. This empowers buyers to choose cost-effective medicines yet offer the same results.

    #3. Cholinesterase inhibitors

All cholinesterase inhibitors have been authorized to treat the mild to moderate symptoms of Alzheimer’s disease (AD).

    Other cholinesterase inhibitors are —

    ●
    Rivastigmine (Exelon®) and Exelon patch

    ●
    Donepezil (Aricept®)

    ●
    Galantamine (Razadyne®)

These medications inhibit acetylcholinesterase, the enzyme that breaks down acetylcholine. One of the substances that facilitates communication between nerve cells is acetylcholine. Some of the signs of Alzheimer’s disease are thought to be caused by low levels of acetylcholine, according to researchers.

These drugs boost the amount of acetylcholine in the brain by inhibiting the enzyme. This improvement in some memory issues and decrease in some behavioral symptoms that are seen in Alzheimer’s disease patients are thought to be the results of this increase.

These drugs don’t treat Alzheimer’s disease or stop it from getting worse. These medications frequently cause nausea, diarrhea, and vomiting as adverse effects. Some users may have hunger loss, sleep disorders, or disturbing dreams.

    Managing behavioral changes

Some of the frequent behavioral signs of Alzheimer’s disease can be managed with medication. Antidepressant medications, for instance, can treat sadness, anxiety, restlessness, and violence. Agitation can be managed using anti-anxiety medications. Aggression is occasionally treated with anticonvulsants.

Hallucinations, agitation, and paranoia can all be treated with antipsychotic medications. Confusion and dizziness are some of these medications’ side effects, which might raise the risk of falling. Because of this, these medications are normally only used briefly when behavioral issues are severe and only when safer and other non-drug therapies have been exhausted.

  • Generic Medicines in India: Lack of Popularity and the Road Ahead

Generic Medicines in India: Lack of Popularity and the Road Ahead

Generic Medicines in India: Lack of Popularity and the Road Ahead

Many generic pharmaceuticals are produced in India and sent to other countries. The Indian pharmaceutical industry is catering to 5% of all medication in the United Kingdom and 40% of all generic drug demand in the United States. now let’s know about Generic Medicines in India: Lack of Popularity and the Road Ahead.

A doctor may prescribe medicine by its brand name or its generic name. The brand name of a drug is the name given to it by the medicine’s developer who holds the patent. The generic name is a different label for the same medication content.

What is generic medicine in India?

The World Health Organization (WHO) defines generic drugs as pharmaceutical products used in place of the originator brand product (that holds a patent). Such medicines require no license from the patent holder to manufacture and market the medicines once the patent expires.

Post the patent expiry, other generic medicine companies may produce and market the same medicine under an international nonproprietary name — often at a lower price. This generic drug overcame the cost barrier associated with patenting costs. Moreover, Indian law enforces prescribing generic medicines to physicians and pharmacists to sell the same.

    Generic Medicines vs Branded

Lack of branding is one of the key reasons why generic medicines are not famous in India. Brand names matter in India — retail goods, electronics, groceries, and consumer durables. And even in the generic drug business, brand names still hold the major sales. On the other hand, branded medicines are leveraging their power of branding through different industry channels and advertisements to create an image that branded medicines have better quality successfully.

Unfortunately, people fall prey to such agendas and prefer branded medicines only to pay premiums for no reason. Some drug brands have a well-known association with their treatments. The brand names have grown to mean the same thing as drugs. Dolo is a fever-reducing medicine in India, but its primary content is paracetamol. Here, people would ask for Dolo and not paracetamol or fever-reducing medicines.

    Importance of Generic Medicines

People need to become more familiar with generic drugs because pharmaceutical companies are using the power of branding to create an unnecessary divide between branded and generic medicines. The branded drugs are marketed to physicians in a manner that creates a wrong perception that branded medicines are better. It also incentivizes the medical lobby — drug stores, private hospitals, local clinics, etc., by offering a share of the sale.

If patients are prescribed branded medicines by certain physicians or hospitals and buy them from a nearby store, the doctor gets the margin from every sale. Therefore, doctors’ endorsement of generic alternatives to brand-name drugs is crucial to increasing their usage. The health ministry has required all doctors nationwide to use generic medicine names.

    Generic Medicines in India

Indian Government Initiatives for Promoting Generics

The Indian government launched the “Jan Aushadhi” initiative in 2008 under the Department of Pharmaceuticals. This initiative aimed to establish government-supported retail stores to sell generic versions of high-quality medicines. It successfully established Jan Aushadhi outlets, where pharmacy stores only offer generic brand medicines.

This program aims to enhance people’s quality of life by lowering medical expenses. It offers high-quality generic medications at lower prices. It also spreads information about the effectiveness of generic medicines amongst physicians who prescribe more generic drugs. The Bureau of Pharma Sector Undertakings (BPSU) acts as the implementing agency, assisted by Central Pharma Public Sector Undertakings (CPSUs) and Janush Aushadhi.

In an amendment to the code of conduct for doctors made in October 2016, the Medical Council of India recommended that all doctors prescribe drugs with clearly legible generic names. And they would do so in a way that encourages generics. The Indian government is mulling and considering whether to enact legislation requiring generic drugs in patient care.

    Common challenges to overcome

The lack of strong regulatory criteria for the amount of medicine in its generic counterpart and its allowable contents has been a major factor for physicians’ (and patients’) In contrast to their cheaper generic counterparts, branded generic medications command a higher price at retail pharmacies. On the other hand, it’s important to know that generic versions of popular brand-name pharmaceuticals work just as well. Research, processing, and production all affect the final product’s quality.

As long as the manufacturers follow precautions to ensure product quality, these generic pharmaceuticals are just as effective in treating patients as the branded treatment. By strictly adhering to GMP norms, pharmaceutical manufacturers may become more accessible to the average person financially. Pharmaceutical equivalent reference products must be therapeutically equivalent to the standard reference product through bioequivalence studies as mandated by regulatory bodies.

In vitro dissolution studies are useful for demonstrating drug-product equivalence in some cases. The lack of strong regulatory criteria for the amount of medicine in its generic counterpart and its allowable contents has been a major factor for physicians’ (and patients’) lack of faith in generic pharmaceuticals.

Possible Solution in Generic Medicine in India

There are 4 Es that help promote effective generic utilization: education, engineering, economics, and enforcement.

Education: This involves creating initiatives that help spread information about generic prescribing to have an impact. Mostly, this is done by leveraging media publications.

Engineering: Prioritizing production measures to drive medication, quality of production, and with supply agreements. This ensures quality production and wider reach.

Economics: This is done by providing positive and negative incentives to physicians and patients. Customers will be saving costs and be shown how physicians benefit from prescribing branded medicines.

Enforcement: It involves keeping regulations in place of law enforcement, which may include obligatory generic substitution regulations that pharmacists and manufacturers must follow.

    Is Generic Medicine Safe?

Quality assurance by the government for any form of pharmaceutical manufacturing is essential. Many international agencies are accusing India of producing low-quality medicines that lead to the death of children in Gambia. The Drugs Technical Advisory Board (DTAB) has proposed implementing barcodes or QR codes for 300 brands of drug products sold in India. The barcode information will include UPC, the name of the active pharmaceutical ingredient (API), the brand name (if any) along with the following:

    • Manufacturer’s name and address
    • Serial shipping container code
    • Batch number & size
    • Manufacturing date
    • Expiry date
    • Manufacturing licence or the import license number
    • Any necessary storage instructions.

Who Started Generic Medicines in India?

Eradicating unawareness around generic medicines also requires well-designed survey studies that consider patients, community pharmacists, and physicians. Usually, generic medicines are perceived as drugs created for the low-income group. Many high-income groups often consider generic medicines a substandard product made for the marginal and poor. And this social perspective needs to be added across government communication. Therefore, future advertisements and communications should also target such groups, goal should be to infer a general picture relevant to generic drug prescribing and buying attitudes.

This includes the role of Janaushadi in increasing the reach of generic medicines to penetrate deeper into the rural market. Even those who can afford medicines in such a market are deprived of them because they lack access to medicines. Therefore, Jan Aushadi stores leverage information technology that enables an end-to-end supply chain system with a Point-of-Sale (PoS) application.

This helps determine the stock and expected availability of products so that people can avoid visiting bigger cities to get medicines. Such an approach includes building a stronger system comprising warehouse management, multiple distributors strategically placed around the nation, and replacing expired medicines.

Next, healthcare providers and government officials must provide drug costs at reasonable levels. An essential instrument in the fight against rising healthcare costs is the introduction of generic equivalents. More and more patents are expiring, and the market for generic copies of pharmaceuticals is projected to continue to see rising sales.

Are Generic Medicines Effective in India

The Pharmaceutical and Medical Devices Bureau of India (PMBI) is responsible for implementing government schemes and promoting generic medicines through a wide range of media. It includes running TV ads, banners on auto rickshaws, FM radio and cinema halls ads, flex banners on railway stations and state transport bus stands, etc. PMBI routinely informs the public how to best use Jan Aushadhi generic drugs through sites like Facebook, Twitter, Instagram, YouTube, and others.

The Bureau also organizes seminars and workshops to spread awareness about the scheme. Moreover, the 7th of March is celebrated as Jan Aushadhi Diwas to create awareness about the program.

There needs to be more understanding between drugstore employees and community pharmacists. Bridging such a gap help promote generic medications amongst everyone, even in tier-I and metro cities. A better understanding of generic drugs amongst customers is crucial for replacing branded drugs. Moreover, recent government interventions, as mentioned above, are boosting the distribution of generic drugs. There are more than 8500 JAS in India across multiple states. Also, the government is targeting 10,500 such stores by 2023.

The time has come to redefine how customers buy medicines in India, how they are distributed, and how physicians prescribe those. When treating the underlying cause of the disease, the original content prescribed to patients is equally effective, regardless of whether they are branded or generic. Since generic drug products greatly impact healthcare costs, medical professionals and patients need a thorough education.

    Conclusion:

Government health authorities can also incentivize purchases by introducing loyalty discounts on generics. At Medkart, we drive awareness around generics through in-store walk-ins, marketing, and other efforts. We also encourage customers to switch to generic alternatives and save expenses by ordering generic medicine online in India.

  • Reasons Why Generic Medicines are Not Prescribed to the Patients

Reasons Why Generic Medicines are Not Prescribed to the Patients

Why don’t doctors prescribe generic medicines?

To understand the reasons behind not getting enough ‘generics’ prescribed by doctors, you first have to know the generic medicines scenario in India. Ours is a country that stands as a global market leader in generic drugs as it exports to countries like the United States, Japan, and African and European countries. Generic drugs have the same chemical composition as marketed drugs. However, they are sold under chemical names that are not open to the public. Enabling the use of generic drugs rather than expensive branded medications is probably one of the most effective ways to reduce medical expenses as it benefits a large part of the population. Still, the majority of people are sold branded medicines. Why?

    Free Doctor Consultation – Book

Most of you may wonder if there is any difference between generic and branded medicines. The generic drugs are cheaper than branded ones simply because the manufacturers don’t have the resources to create and market the newly-crafted medicine. When a company launches a new medication, the company has already invested truckload of money on conducting drug research, production, and promotion/marketing activities. Most of such advertised and well-marketed medicines will fall under the ‘branded’ label.

Now, from where will they earn the additional cost of marketing and promotions? Of course, the consumers have to bear it because the manufacturers also need to make a profit. And that’s when doctors come to play as an agent. Doctors work on commission-basis, getting a cut from the medical representatives who market and endorse a drug for the company they work for.

Now, these medical representatives are also getting a share of commission as they sell the same to doctors as ‘gifts’ who, in turn, recommend the same to their patients to get their share of the cut, making patients more vulnerable due to the former’s authoritative position. And since patients trust doctors blindly, follow every word they say, the doctors leverage their position to prescribe branded medicines. The problem is also with the patients as most of them are not aware of the existence of generics and don’t even bother asking about it to doctors.

Apart from the commission aspect, general physicians also play a major role in taking the branded market forward and shooting up the costs. The more the doctors recommend, the more likely it is to be recommended to the people, and the company can then exercise a monopoly. Customers are equally responsible for contributing to an already-existing large pie by blindly trusting the “brand name,” be it product like shoes, jeans, mobile phones, or even medicines where brand names like Crocin highly used for Paracetamol tablets.

Another reason is that the patients lack the awareness regarding generic medicines as many feel that cheaper medicines are not as effective as the expensive, branded ones.

Many chains of hospitals are closely associated with the pharma companies where they earn a high commission on selling the drug manufactured by those companies within their premises. Such an ecosystem breeds a positive attitude towards branded medicines, where even the patients feel that branded medicines are only effective ones. That’s why even doctors feel that not prescribing branded medicines would mean patients doubt their diagnosis, or if they are given another cheap medicine the second time, they won’t appreciate it.

Of late, the health ministry in India decided that all physicians must mandatorily prescribe generic medicine. But, we think it will take a long time to implement this one. Till then, it is up to the buyers to stay aware of the generics and choose their way. The best option is to reach Medkart with the doctor’s prescription and ask about it. Our representatives will make you understand the contents of the prescribed medicines and its effect on your body, thereby offering you generic options and helping choose one. Since the beginning of the launch, we have been following this practice and spreading awareness about generics to people so that they can make the right choice about what they consume.

Why do doctors prescribe brand-name drugs?

Marketing and promotion: Pharmaceutical companies often engage in marketing and promotional activities to create awareness and familiarity with their brand-name drugs among healthcare providers. This can include providing samples, educational materials, and financial incentives to doctors, which may influence their prescribing practices.

Drug availability: Sometimes, due to supply chain issues or other factors, generic versions of certain drugs may not be readily available in the market. In such cases, doctors may have no choice but to prescribe the brand name version of the drug to ensure that their patients have access to the medication they need.

Paracetamol Brand Name and Generic Name

Generic Paracetamol and Branded Paracetamol – The brand name for a drug called acetaminophen, Tylenol, or Panadol is Paracetamol. It is widely used as a minor pain reliever and a fever reducer, among other things. For many years, people have used this drug. Continue reading to learn more about Paracetamol brand name and Paracetamol generic name and Know about Paracetamol Brand name and Paracetamol Generic.

    Background of Paracetamol Brand

The development of this non-addictive painkiller began in 1893. No one had used it in a clinical setting before. It became commercially usable in 1950 in the United States. In 1956, Australia began exploiting it for profit. Initially marketed under Triagesic, this medication contained aspirin, caffeine, and Paracetamol.

The 1950 original introduction was discontinued from commercial use by the producers until 1953. The Sterling-Winthrop Company first sold it under the brand name Panadol. In 1955, Children’s Tylenol Elixir was the brand under which McNeil Laboratories marketed the product in the US. Up until 1959, Paracetamol could only be obtained with a prescription. Then it changed to an over-the-counter drug.

Under the brand name Panadol, Frederick Stearns & Co. started distributing this medication in the UK in 500-milligram pills in 1956. The drug’s popularity rapidly increased between the 1960s and the 1980s.  Many generic variants of Paracetamol are accessible since all patents have expired.

    What is the Composition Paracetamol?

Paracetamol is an analgesic and antipyretic medication used to treat mild to moderate pain and fever temporarily. It is frequently used alone and as a component in treating the flu and the common cold.

Paracetamol and acetaminophen are the same medication. While the International Nonproprietary Name (INN) system uses the generic name Paracetamol, the United States Adopted Names (USAN) system uses acetaminophen.

The drug is commonly known as either Paracetamol or acetaminophen, and the name varies by country.

In Europe, Australia, New Zealand, and India, the medication is called Paracetamol, while it is referred to as acetaminophen in the United States, Canada, and Japan.

Generally, regardless of location, a drug’s INN and USAN generic names remain identical.

The precise mechanism of Paracetamol’s action is unknown, but it is believed to function by inhibiting the brain’s chemical messengers that alert us to pain and control our body’s temperature.

According to a study, paracetamol prevents the creation of prostaglandins by the body to address disease and injury. It may also impact pathways such as cannabinoids, serotonergic, opioid, nitric oxide, and others.

Although Paracetamol was created for the first time in 1878, its use took off in the 1950s. One of the most often used painkillers in the world today is Paracetamol. There are two types of medicines — generic and branded ones. While the composition for both tablets remains Paracetamol, the prices would differ, with the generic being less expensive

    Paracetamol in Combination Medicines

Many cold and flu medications combine paracetamol with other active ingredients such as phenylephrine and chlorpheniramine to provide multi-symptom relief. For example, Cheston Cold Tablet and Ultracet tablet 15s contains a combination of paracetamol, phenylephrine, and chlorpheniramine, offering relief from headache, nasal congestion, and fever. While the core component — paracetamol — remains the same, these combination drugs address a wider range of symptoms than plain paracetamol alone.

Difference between Generic Paracetamol and Branded Paracetamol

    1. Generic Paracetamol

Cost: Generic paracetamol is typically more cost-effective as it does not involve the same research and development expenses.
Name: Sold under its generic name, “paracetamol” or “acetaminophen,” without a specific brand attached.
Packaging: Generally features simpler packaging, often displaying the generic name and necessary information without elaborate branding elements.
Development: Developed after the expiration of patents on the branded version, allowing multiple manufacturers to produce it.

    2. Branded Paracetamol:

Cost: Branded paracetamol tends to be more expensive due to the investment in research, development, and marketing.
Name: Sold under a specific brand name (e.g., Tylenol, Panadol) with recognizable logos and design elements.
Packaging: Features distinctive packaging with a unique brand name, logo, and design, reflecting the manufacturer’s marketing strategy.
Development: Introduced to the market after extensive research, development, clinical trials, and regulatory approvals, often protected by patents for a certain period.

    All Paracetamol Brand Names

Tylenol, Panadol, Crocin, Calpol, Anacin, Mapap, Feverall, Dolo, Acimol, Paracip

    How does Paracetamol work?

This medication’s specific mechanism of action is uncertain. But there are hundreds of definite possibilities out there. Scientists and medical professionals think this medication affects the central nervous system (CNS), specifically the brain and spine.

This medication has a minimal analgesic effect. It aids in reducing the strength of the pain signal your body delivers to your brain. These substances may make you feel hotter or make your pain worse.

    How to take this drug?

An adult typically uses one 500 mg pill every four to six hours, with a maximum consumption of 2000 mg in 24 hours. At least four hours should pass between doses. Additionally, the working time is up to one hour.

Parents permit children to use Paracetamol, determining the appropriate amount based on weight, typically using 15 mg for every kilogram. Therefore, the recommended dosage for your youngster, who weighs 15 kilograms, would be 225 mg every four hours. Additionally, take only the advised dosage in 24 hours. You can get types of generic drugs as well.

    Side effects of Paracetamol

A few of the negative effects could be uncommon but severe. If you have any of the following adverse effects, especially if they persist, speak with your doctor.

    ●    Swollen Facial Features

    ●    Liver Damage

    ●    Feeling Of Sickness

    ●    Skin Reddening

    ●    Allergic Reactions

    ●    Shortness Of Breath

    Precaution

Inform your doctor about your current prescriptions, over-the-counter products (such as vitamins, herbal supplements, etc.), allergies, pre-existing problems, and current health conditions before using this medication (e.g. pregnancy, upcoming surgery, etc.).

You can be more susceptible to the drug’s side effects if you have certain medical conditions. The market has a range of generic medicines for Paracetamol, therefore, in case of preexisting conditions that require regular medicines, consult the doctor before taking one.

When consuming, follow the doctor’s advice or the directions on the package insert. Your condition will determine your dosage. Inform your doctor if your problem continues or worsens.  Avoid using Paracetamol if you have a sensitivity to it. Avoid taking Paracetamol if you frequently drink alcohol.
